The guy who runs the place is Jack Smrekar in Florence, Oregon at 1-541-997-6349.

You send him the picture you want show via e-mail(the more pixels the better) and make it 10.65 inches wide. Then send him via snail mail the fly you caught the fish on. He takes your image and puts in on acid free paper and then below the picture of the fish, he puts the fly in a shadow box. The finished size of the frame is 17" x 17". He charges $150 which includes shipping(at least to me in Oregon). The finished product is very professional.
Recently I had him do both a ten pound New Zealand rainbow and a 29" Rogue summer steelhead. His web site will show you several examples of his work. He calls them Trophy Fly Plate
